First Visit papers

If you are a new patient and would like to get a head start on your paperwork, you are welcome to do so.  Using the link below, simply print the forms, fill them in and bring them with you to your first consultation.  If possible, please also bring your current RX medications, your insurance card, and a government-issued ID. You must have a referral and be approved by our physicians to be seen as a new patient.
